Genesis Home supports three programs that serve homeless families in the Durham community. Since 1989, Genesis Home has provided quality transitional shelter and support for families through our original program, Family Matters. Genesis Home’s Turning Point program, established in September 2006, provides rental assistance and supportive services for five families in permanent housing headed by an adult with a diagnosed disability. Modeled after the Housing First initiatives outlined in Durham’s 10 Year Plan to End Homelessness, this program is predicated upon the belief that when a family’s housing is stable and secure, the family can begin to regain self-confidence and control. Circles of Support matches formerly homeless families and individuals with Support Circles (local congregations and civic organizations) to provide support and assistance for a period of 12 months.
Genesis Home actively assists each resident with achieving their specific outcomes by:
· Providing Individualized Case Management - Immediately upon arrival at Genesis Home, residents begin meeting with case management staff who offer the support and resources needed in order to identify the root cause of their homelessness, overcome obstacles, and build upon their strengths.
· Seeking Financial Assistance – In order for each family to increase its income, Genesis Home’s case managers help the residents enroll in public assistance programs, assist with finding employment and educational opportunities, and teach them to use their income to pay down any debt that may prevent them from finding permanent housing.
· Offering Self-Help Opportunities – Genesis Home’s case managers, volunteers and partner agencies provide training and counseling on issues such as budgeting and finance, anger management, overcoming addiction, domestic violence and abuse, encouraging appropriate familial relationships, and parenting.

This notice describes the privacy policy of Genesis Home regarding clients’ protected personal
information (PPI) that resides in the Homeless Management Information System
(HMIS) database. Genesis Home collects personal information only when it is
appropriate and/or required. Genesis Home may collect information for these
purposes:
a. to
provide or coordinate services to clients
b. to
locate other programs that may be able to assist clients
c. for
functions related to payment or reimbursement from others for services that we
provide
d. to operate our organization, including
administrative functions such as legal, audits, personnel, oversight, and
management functions
e.
to comply with government reporting obligations
f. when required by law We assume that residents of our
program agree to allow us to collect information for the purposes listed above.
Client consent is implied as a precondition for program entry. Clients can
inspect their personal information in their case file. Clients can ask us to
correct inaccurate or incomplete information. Clients can ask us about our
confidentiality policy and practices. Clients are also able to determine the
accessibility of their non-protected personal information to other providers in
our community. We are happy to respond to any questions and concerns that you
may have about HMIS.
